Professional Medical Associates offers in-house Chronic Care Management services for patients with multiple chronic illnesses. We only see you 4-6 times a year, but we know you deal with your chronic illnesses every day. CCM is a way for us to make sure you have the resources you need to help you stay healthy. CCM Patients still see their doctor for regular visits as normal, but while out of the office they will contact their CCM nurses for refills or any other needs. Talk to your doctor if you think you could benefit from CCM.
